TFAST® was purposely designed with its 5 views by using gravity least-dependent Chest Tube Site (CTS) views bilaterally for pneumothorax (PTX) because PTX can be unilateral. The patient being upright is best because air rises, an important principle to understand that is dependent on patient positioning. The gravity-dependent PeriCardial Site (PCS) views are used for fundamental echocardiography from the left and right sides, and their use for PCE and PE because fluid falls into gravity dependent regions we call “pouches.”
Lastly, TFAST® has the Diaphragmatico-Hepatic (DH) View that is used for the “Racetrack Sign” of PCE, PE, lung surface interrogation along its pulmonary-diaphragmatic interface, and volume status via the characterization of the caudal vein cava (CVC) and hepatic veins, and its use for ascites and soft tissue abnormalities of the lever and gallbladder, as well as gastric characterization by rocking causally.
